FORMS -
All new patients (Except DOT) will need to fill out a Welcome Packet. If you need a CDL/DOT Physical, you will need to fill out JUST the DOT History Form. If you have insulin-dependent diabetes and need a DOT card, fill out and bring the IDDM FORM with you.
If you are a Worker's Compensation patient, You will need to fill out Welcome Packet as well as an 827 Form.
